Tennis Entrance Forms

The Individual Tennis Tournament Entrance Form and Team Tennis Entrance Form(s) shall be submitted to the Entry Level Tournament Director and the TSSAA Office via the TSSAA Portal at least one week prior to your entry level tournament. If you already submitted a paper version, it will be fine and accepted this year. In 2026, all forms will need to be completed in the digital format on the portal.

Once submitted and your school has played in their first post-season tournament, the form(s) cannot be changed as they will carry over to the next level tournament. The only adjustment to this form will be if a player is injured or sick. You can remove the injured/sick player from the line-up and move everyone else up in "order of ability". If the injured/sick player recovers for the next match, you can put the player back in the original line-up from the form.

Head Coaches should have received instructions for filing these forms in the TSSAA Portal via email. If you did not receive the instructions, please check your Spam or Junk Mail folder. You can also email the TSSAA office, tssaa@tssaa.org, requesting that the instructions be emailed to you again.

Athletic Directors can also submit the forms and access them under the Forms Menu of the TSSAA Portal.

Each form can only be submitted once. If a correction needs to be made, the school athletic director will need to delete the form (under Submitted Forms) before a corrected version can be submitted.

Once you complete the form, an email will be sent to the TSSAA, you, and your athletic director. If the TSSAA has the correct tournament director information, a separate email will be sent to the tournament director with a link for them to access all the forms for their tournament. However, you can forward the email receipt you receive to them as well.
